Add a header to begin generating the table of contents

With the upcoming new version, I am taking the myCred Codex offline over this weekend in order to update the documentation.
Besides adding information about the new features coming in 1.4 I am also moving installation guides from the add-ons pages to the codex. For developers, I am also adding documentation on all myCred APIs to help you build modules, hooks, gateways, etc.
If you are a PHP / WordPress developer I will also be introducing the option to list your myCred profile on the Codex page under a new “Hire a developer” page. More information on how this will work will be announced next week.
